Unhath, along with her twin sister Reluhantis, was one of the marilith consorts and majordomos of the demon lord Graz'zt. Much like her sister, she rarely left the Prince of Pleasure's side, and operated on the Material Plane by possessing mortal vessels. One of her identities was the lamia queen Ereshkigal in Unther.

Tyranny of Dragons
Unhath was overseeing Graz'zt's forces in Unthalass, alongside her daughter Ninaza, by using the body of the lamia queen Ereshkigal, who was also the empress of the god-king Gilgeam. Her true identy, however, was revealed when she took an intense interest in Nayiss Ourobori, whose existence had apparently been kept from her by Reluhantis.
While negotiations between Tymanther and Unther had started to deteriorate, Unhath had proclaimed that the rest of those present could leave—though Nayiss would be kept. Given such a situation was unacceptable, Nayiss, Ourobori Jarkul, the Spellforged, and the rest of the Thymari forces began to fight against the swarming forces.
Unhath took the lead on the attack, and attempted to break down whatever cage Nayiss had secured around her true nature. However, Jarkul and Hostu Twinsmiles continually interfered with her attempts.
Soon after Ninaza arrived on sight to back her mother, Nayiss struck the killing blow. With the final breath of her vessel, Nayiss proclaimed that she could return to the Abyss with the knowledge she had failed.
