As in Tasha's Cauldron of Everything with the 5-foot limiter removed. Damage also increases again at 23rd level and 28th level.

Changelog

Altered Attack must be made with a melee weapon, but it does not need to be within 5 feet of the caster, thus this spell works with reach weapons.
Added epic level damage progression.
